Instructions
This
agreement must be signed by the person responsible for individual or
chapter viewing of the named videotape. Please print this document
from your browser, read the agreement, fill in the required
information, and sign the agreement.
The
completed and signed agreement along with two checks should be sent to
the Visual Materials Development Chairman. Both checks should be made
out to ANG, the first one for $11.50 to cover postage and handling, and
the second check for $20 as a deposit.

Video/DVD Rental Agreement
As the person responsible for individual or chapter viewing of the
videotape listed below, I certify that:
- I
understand the videotape/DVD and any accompanying educational
material are copyrighted, and I will abide by copyright laws. My
obligations include but are not limited to the following:
· I will not copy the videotape/DVD .
· I will retain possession of the videotape/DVD and educational
material during the rental period, and I will not loan them to
another party. I
will play the videotape/DVD only on a machine in good working
condition so that the videotape/DVD will not be damaged during
use. I
or the chapter to which I belong will not charge for viewing of
the videotape/DVD. I
understand that the videotape/DVD and any associated educational
material have left ANG's possession in good condition. In case of
either visible or concealed damage upon arrival, I agree to notify
the ANG Visual Materials Development Chair within five days, and I
agree to keep the shipping carton and all the packing material
until the shipping company inspects the damage claim. I
am responsible for all damage to or loss of the videotape/DVD and
accompanying educational material once I receive them. In the
event of loss or damage, I agree to pay for replacement of the
videotape/DVD and any accompanying educational material.I
will return the rented videotape/DVD and accompanying educational
material so that they are in the hands of the Visual Materials
Development Chair on or before the due date, which is four weeks after the viewing date listed below. I understand
that failure to return the videotape/DVD and accompanying material
on time will result in forfeiture of the $20 deposit.
- I
will return the videotape/DVD and accompanying educational
material with shipping costs prepaid and insured for $51. I will return them in the original shipping container,
unless it was damaged upon arrival.
